IMPORTANT NOTICE FOR ALL PATIENTS WITH ANTHEM / BCBS INSURANCE:
Mount Sinai and Anthem have been negotiating their contract renewal. If they do not come to an agreement, then as of January 1, 2026 Mount Sinai will be out of network, as well as our sites (MFM Associates, Carnegie Imaging, Carnegie Women’s Health, Mount Sinai FEU, Mount Sinai 60th Street). Often, when negotiations like this happen, they do come to an agreement and coverage is uninterrupted, but we are doing the following in the interim:
If you are pregnant and received services from us in 2025:
- You will continue to have coverage with Anthem / BCBS through the end of your pregnancy
- To ensure this, you will need to fill out a continuity of care form (CoC).
- Please reach out to your insurance to obtain a CoC form if you have not done so already.
If you are not pregnant, or you are pregnant but were not seen in one of our offices in 2025:
- If/when Mount Sinai and Anthem come to an agreement, we will submit our services to Anthem / BCBS in-network, as we would normally do. You will only be required for the contractually required co-pays / co-insurance / deductibles, as per usual.
- If Mount Sinai and Anthem do not come to an agreement, we will submit your claim to Anthem / BCBS as out of network services. In this scenario, YOU WILL NOT BE REQUIRED TO PAY OUT OF POCKET EXPENSES FOR OUT OF NETWORK CHARGES.
Again, we are hopeful and confident that this situation between Mount Sinai and Anthem will resolve in the near future. We are sorry for any inconvenience this causes you.

Not all plans under all payers listed are accepted. Please contact our billing department prior to making your appointment to assure we participate with your individual plan.
Charges for copayments, deductibles and co-insurances are collected upon registration at the time of service.
Please note that if your insurance is on this list, it does not automatically ensure that you will be fully covered for your visit(s). It is the patient’s responsibility to find out the terms and conditions of their insurance plan. The list of insurance plans we accept is subject to change. Please call to confirm a listed carrier or inquire about another plan that might not be posted here.
If your insurance company is not on this list, or if you do not have health insurance, please note that payment, in full, is expected at the time service is provided, unless prior arrangements have been made with the office.
All patient charges are the patient’s responsibility. Therefore, it is recommended that you call your insurance company directly if you are unsure whether this appointment will be covered.
We accept personal checks, MasterCard, Visa, and American Express. All patients should bring their insurance identification cards (if applicable), referrals/authorization numbers (if applicable), and any other pertinent information at the time of their appointment.
Referrals and Pre-Authorizations
If a referral or a pre-authorization number is required for this visit, it must be obtained by the patient prior to their appointment. Please contact our billing staff in advance for instructions to follow when contacting your insurance carrier for pre-authorizations.
